Individualized Living Options (ILO)

If you are contemplating to make decisions about where and with whom you live, and have got funding in your NDIS plan for Individualised Living Options (ILO), we as an ILO provider would help with you to make this happen.

ILO Stage 1- Exploration and Design

We Will Work With You To:

  • Explore where and how you want to live, including helping others around you understand what you want and this will work for you.
  • Understand your strengths and capabilities and learn what you can do to build your skills through your NDIS individual support plan
  • Choose where you live, who you live with and how you are supported, find out what is possible and assess and manage any risks in your preferred way of living.
  • Help you design and make decisions about your ILP

At the end of Stage 1, we would have reached a decision point about your preferred home and living arrangement and the package of supports

ILO Stage 2- Supports

Under This Stage We Will:

  • Assist in the completion of the Service Proposal Form.
  • Develop an ILO Service Proposal
  • Facilitate the drafting of agreements between you and supports, supports and providers, and you and provider.
  • Undertake dispute resolution and problem solving
  • Monitor and adjust the ILO arrangements as required.
